SPADI Shoulder Index: Pain & Disability Subscales, Scoring Formula and MCID
In short
The Shoulder Pain and Disability Index (SPADI) measures pain and functional loss across rotator cuff disorders and adhesive capsulitis. Learn scoring formulas and MCID.

What Is the Shoulder Pain and Disability Index (SPADI)?
Developed by Kathryn Roach and colleagues in 1991, the Shoulder Pain and Disability Index (SPADI) is a widely applied, region-specific questionnaire designed to assess shoulder pathology.
Consisting of 13 items divided into two subscales, SPADI is particularly sensitive in evaluating rotator cuff tendinopathy, subacromial impingement, and adhesive capsulitis (frozen shoulder).
Subscale Breakdown and Scoring Calculation
Each item is rated on either an 11-point visual analog scale (VAS) from 0 to 10 or a numeric rating scale (NRS) from 0 (no pain / no difficulty) to 10 (worst pain imaginable / so difficult required help).
Both subscales are transformed to a percentage, and the overall total score is calculated as the average of the two percentages.
| Subscale | Number of Items | Max Raw Score | Percentage Calculation Formula | |
|---|---|---|---|---|
| Pain Subscale | 5 items | 50 points | (Sum of completed pain items / Max possible pain points) * 100 | |
| Disability Subscale | 8 items | 80 points | (Sum of completed disability items / Max possible disability points) * 100 | |
| Total SPADI Score | 13 items | 130 raw points | (Pain Subscale Percentage + Disability Subscale Percentage) / 2 |
MCID and Clinical Recovery Benchmarks
Research by Roy et al. and Paul et al. establishes a Minimal Clinically Important Difference (MCID) of 8 to 13.2 points for the total SPADI score across outpatient shoulder rehabilitation cohorts.
In frozen shoulder recovery, an initial SPADI score of 70 to 80 percent commonly reduces to under 25 percent following progressive hydrodilatation, capsular stretching and rotator cuff strengthening.
- Pain items: Worst shoulder pain, pain lying on involved side, reaching for high shelf, touching back of neck, pushing with involved arm.
- Disability items: Washing back, putting on undershirt/sweater, putting on button-down shirt, putting on pants, placing object on high shelf, carrying 5 kg object, removing object from back pocket.
- Missing responses: A subscale can still be scored if the patient omits no more than 1 item in the pain subscale and no more than 2 items in the disability subscale.
Frequently Asked Questions
Can SPADI differentiate between adhesive capsulitis and rotator cuff tears?
No. SPADI is an evaluative outcome tool, not a diagnostic test. However, frozen shoulder patients typically score much higher on disability items involving internal rotation (washing back, reaching into back pocket).
Does a higher SPADI score mean better or worse shoulder function?
A higher score indicates greater pain and disability. A score of 0% indicates completely asymptomatic function, while 100% indicates complete shoulder disability.
How long does SPADI take to complete?
Patients usually complete the 13-item questionnaire in 3 to 5 minutes.
